Monash IVF has transferred the wrong embryo into a Melbourne woman.
In a statement to the ASX on Tuesday, Monash said the incident was under investigation.
The provider has apologised to the couple involved.
‘Commencing immediately, Monash IVF will implement interim additional verification processes and patient confirmation safeguards over and above normal practice,’ the statement said.
‘Monash IVF has extended its sincere apologies to the affected couple, and we continue to support them.’
It follows Monash IVF’s admission last month that a mistake at its Brisbane clinic led to a Queensland mother unknowingly delivering another couple’s baby.
The Brisbane mistake had occurred at least a year earlier but was only discovered when the woman’s embryos were being relocated to another clinic.
